CANCELLATION POLICY

C2MECC, LLC understands that things happen in our lives that are beyond our control. When these things happen, you are requested to notify this office within 24 hours of your appointment to cancel or reschedule. You will be charged $95.00 if you fail to cancel an appointment with C2MECC, LLC within 24 hours. Payment and keeping appointments are therapeutic goals that are just as important as the goals developed during your therapeutic journey. You also need to know YOU will be billed for your non-canceled appointments as insurance companies nor Medicaid pays for these.

C2MECC, LLC hours of operation are Monday through Thursday 9 a.m. – 6 p.m., Fridays 9 a.m. to 12 noon and weekends are by appointment only. C2MECC, LLC does not answer the phone while in session however, you will be able to access voice mail. Voice mail is checked through the days of operation and calls are returned as promptly as possible. If your call is of an emergency nature, please contact your primary physician or go the hospital emergency room for assistance.

COMPLAINT PROCESS

C2MECC, LLC is committed to providing the best quality of services available. Your mental health is a serious issue. Should you have a complaint about your therapist or you believe that an unethical practice has taken place, know that you are encouraged to file your complaints or express your concerned with the following:

TERMINATION OF THERAPY

Unfortunately, all relationships of this nature must terminate at some point in time. There will come a time in therapy where you have successfully completed the process and therapy comes to an end. When this happens, it is important for you to know that we will not be able to meet socially, go into business together, etc., even after therapy is terminated. This is for your protection as well as C2MECC, LLC.

YOUR RIGHTS AND RESPONSIBILITIES AS A CLIENT

YOUR RIGHTS:

• To be treated with respect and dignity
• To care that is considerate and respects your personal values and belief systems
• Personal privacy and confidentiality of information
• To reasonable access to care, regardless of race, religion, gender, sexual orientation, ethnicity, age or disability
• To participate in an informed way in the decision making process regarding your treatment planning
• To discuss with their provider the medically necessary treatment options for their condition regardless of cost or benefit coverage
• To an individualized treatment plan and periodic review of this plan
• A competent, qualified professional therapist to carry out your treatment plan
• To receive information about a mange care company’s services, practitioners, clinical guidelines and patient rights and responsibilities
• To voice complaints or appeals about a managed care company or their provider of care

YOUR RESPONSIBLITIES:

• The responsibility to give their provider and managed care company information needed in order to receive appropriate care
• To follow their agreed upon treatment plan and instructions for care
• To participate, to the degree possible, in the understanding of their behavioral problems and in developing with their therapist mutually agreed upon treatment
